Anyone in England can register with a GP surgery. It's free to register.

If you are registering as a family with under 18s, please ensure parents/guardians are registered first.

Please allow up to two working days for your registration request to be actioned. You will receive a confirmation text/email once it has been completed.

Welcome to Farnham Park Health Group!

All new patients must create a Patchs account as this is how we accept and triage your requests. Please click on the link below to create a Patchs account.
Once you have done this, please submit a request for a medication review (if applicable) and health check. All new patients must have a medication review before any medication is issued. Please ensure this is done promptly so that you do not run out of medication before your review with us.

For all new baby registrations, please email frimleyicb.registrationsfphg@nhs.net to book in your 8-week baby vaccinations and mums’ 8-week check.

What is temporary registration?

If you are ill while away from home or if you are not registered with a doctor but need to see one, you can receive emergency treatment from the local GP practice for 15 days by registering with the practice as a ‘short stay’ patient. After 15 days you will need to register as a ‘long stay’ temporary (up to 3 months) or permanent patient.
