If you are suffering with back or neck pain or a joint or muscle injury, you can now self refer to the Physiotherapy Department at Farnham Hospital without seeing your GP first!

You need to be a patient at Dr O'Donnell & Partners and also a registered user of this site to download a copy of the self-referral form from this page (which is only visible to registered users).  Simply complete the form and send, fax or take the form into the Physiotherapy Department at Farnham Hospital.  Once your referral form has been received, you will be sent a letter giving you details of your appointment.

You can also ask for a copy of the self-referral form at our reception.

Please Note: This is not a fast track system, your referral will be prioritised in the same way as all other referrals but it will save you the time of making an appointment to see your GP who then may make a referral for you.
