Preventing identity fraud

The Strategic Health Authority require that all new patients registering with the Practice must produce proof of identity to verify the patient is who they claim to be and are entitled to NHS treatment. Patients are asked to provide original documents at the time of registration which can be any one of the following document types:

  1. Birth or marriage certificate
  2. Medical card
  3. Driving license (preferably photographic)
  4. Passport
  5. Local authority rent card
  6. Paid utility bill
  7. Bank card or statement
  8. National Insurance card.

Registering as a Patient at Dr O'Donnell & Partners
To register as a patient at Dr O'Donnell & Partners, all you need to do is complete a new patient questionnaire (NPQ for patients over 16 and under 16 are available for download from this page) and bring it into the surgery along with your Medical Card.  If you do not have a Medical Card, then you will be asked to also complete a GMS1 form (please double side print on A5 if you can). Because of identity theft, regulations now require that all new patients must provide proof of identity and residence.  

Following changes introduced by the Department of Health in April 2004, patients are now registered with the Practice rather than with an individual GP.  The Practice however operates a system where patients can state a preference for a particular GP depending on whether the GP in question is able to accept more patients.  Where a preference is stated, we prefer patients to remain with that doctor to ensure continuity of care. 

Please note that Drs Susan Clarke and Helen Roberts job share and therefore also share a "preference" list, as such patients will be expected to see either GP.  Dr Clarke works Mondays - Tuesdays and Dr Roberts Thursdays to Fridays.

New patient questionnaire?

We ask all new patients to complete a simple questionnaire for themselves and their children.  This is because not all practices are advanced as Dr O'Donnell & Partners in transferring patient records electronically.  We therefore need to have some basic information to ensure you and your family receive continuity of care while we wait for your notes from your last GP.

Download, print and complete a GMS1 Form (if you do not have a medical card) and a New Patient Registration Form for each member of your family and bring it in with you when you are ready to register:
