Carer's Assessment
The Carers Act (1995) places a duty on local authorities to consider the carer’s needs as well as those of the person for whom they care.
 
You are entitled to an assessment of your own needs as a carer, entirely separate from the needs of the person you are caring for, which could result in the provision or an increase in services.

Do you look after someone?

Carers look after family, partners or friends because they are ill, frail or have a disability. The care which is provided is unpaid.

A carer is someone whose life has changed as a result of looking after someone who is unable to manage due to physical or mental illness, disability, frailty or substance misuse

A young carer is a carer who is under 18 years old. They help in lots of ways like doing housework, keeping an eye on someone, helping them to wash and dress and cheering them up.
 

If you are an adult or young person providing a regular commitment to a relative, friend or neighbour who cannot manage on his or her own, then……

you are a carer and help is available for you

 

Young carers
Carers can be young too you know! How can young carers get help?
 
Ask our receptionist for a registration form. Once you have filled this in and returned it, your GP will then know that you are a young carer. This will make them aware of what you need to make sure you can keep fit and healthy too!
  
You can also call Surrey Young Carers. Through them you will get a chance to meet other young people who may be in a similar situation to you.
  
How you choose to use Surrey Young Carers is up to you, whether its for advice, support or just to have a good time!

Register as a carer

Tell a member of our staff that you are a carer or complete our Carers Form >> and bring it in with you the next time you're visiting the surgery.

Your medical records will be marked so that whenever your GP or other health professionals like our Practice Nurses see you, they will know that you are a carer.

Your health and well-being can be monitored and help given where necessary.
