If you need a blood test your GP or practice nurse will give you a completed blood test form.  Phlebotomy clinics are held in the Outpatient Department at Farnham hospital, Monday to Friday 08:00 to 12:30 and 13:30 to 16:00.   Patients who need fasting blood tests should not eat for 12 hours prior to the test and may only consume water of a morning.
